a. duly certified photocopy of transcript of academic records (university level) b. one recent colored photograph (5cm x 5cm) c. any document(s) attesting to having passed a board examination or to being a scholar of any agency d. application form (duly accomplished) e. two referral forms: one to be accomplished by the current (immediate) superior (or Dean, in the case of a school) and the other to be accomplished by a professor in the student’s undergraduate major |

of the Test of English as a Foreign Language (TOEFL) with a score of at
